{"id":60280,"date":"2020-06-24T14:03:31","date_gmt":"2020-06-24T17:03:31","guid":{"rendered":"https:\/\/www.rioonwatch.org\/?p=60280"},"modified":"2020-07-13T10:32:55","modified_gmt":"2020-07-13T13:32:55","slug":"divergent-methods-and-conflicting-outcomes-in-brazils-covid-19-coverage","status":"publish","type":"post","link":"https:\/\/rioonwatch.org\/?p=60280","title":{"rendered":"As Brazil Flouts WHO Guidance on Covid-19 Case Counting, Belgium Shows a Better Way"},"content":{"rendered":"<p style=\"text-align: right;\"><strong><em><a href=\"https:\/\/bit.ly\/DadosSalvamVidas\" target=\"_blank\" rel=\"noopener noreferrer\">Clique aqui para Portugu\u00eas<img decoding=\"async\" width=\"20\" height=\"20\" class=\"alignright size-full wp-image-23766\" src=\"https:\/\/www.rioonwatch.org\/wp-content\/uploads\/2012\/08\/PT-e1439583827971.png\" \/><\/a><\/em><\/strong><\/p>\n<p><em>This is our latest article on the\u00a0<a href=\"https:\/\/bit.ly\/CoronavirusNasFavelas\" target=\"_blank\" rel=\"noopener noreferrer\">new coronavirus as it impacts Rio de Janeiro\u2019s favelas<\/a>.<\/em><\/p>\n<p><span style=\"font-weight: 400;\">On June 12, Brazil surpassed the United Kingdom, and now holds the world\u2019s <\/span><a href=\"https:\/\/bit.ly\/2x6VM4f\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">second-highest Covid-19 death toll<\/span><\/a><span style=\"font-weight: 400;\">. And yet, amid concerns of widespread <\/span><a href=\"https:\/\/bit.ly\/UnderreportingCovidFavelas\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">underreporting<\/span><\/a><span style=\"font-weight: 400;\">\u2014especially in the favelas and peripheries\u2014Brazil\u2019s true figures may far surpass its official record of over 52,700 deaths and 1.15 million confirmed cases. These concerns have taken on new urgency since the federal government took steps to modify and then restrict the publication of national-level Covid-19 data. The same was also done in the <\/span><a href=\"https:\/\/bit.ly\/3ene3u7\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">municipality of Rio de Janeiro<\/span><\/a><span style=\"font-weight: 400;\">.<\/span><\/p>\n<h3><b>A Brief History of Covid-19 Case and Death Reporting Methodology in Brazil<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">On June 6, the Ministry of Health tried to <\/span><a href=\"https:\/\/bit.ly\/3hxdhx7\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">fundamentally change Brazil\u2019s case and death reporting methodology<\/span><\/a><span style=\"font-weight: 400;\">. The executive body pulled months of data, removing the total number of confirmed Covid-19 cases and deaths since the pandemic\u2019s onset, leaving only a daily total of new confirmed Covid-19 cases and deaths over the last 24 hours.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">Justifying its choice to remove accumulated totals of Covid-19 confirmed cases and deaths, the <\/span><a href=\"https:\/\/bit.ly\/3hxdhx7\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Ministry of Health said<\/span><\/a><span style=\"font-weight: 400;\"> that \u201c[t]he new publishing model for information about Covid-19 will address the current panorama of the disease\u201d because \u201c[i]n the publication, purely, of accumulated cases\u2026 it is very difficult to verify changes to regional, state and municipal scenarios.\u201d <\/span><span style=\"font-weight: 400;\">The removal flew in the face of <\/span><a href=\"https:\/\/bit.ly\/2Y8ctXE\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">World Health Organization (WHO) surveillance strategies guidance<\/span><\/a><span style=\"font-weight: 400;\">, obscuring comprehension of the overall progression and evolution of the pandemic.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The Ministry also switched from compiling data of Covid-19 deaths according to the date on which the death was recorded, <\/span><a href=\"https:\/\/bit.ly\/3fa00Z9\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">as is common in the majority of countries<\/span><\/a><span style=\"font-weight: 400;\">, to compiling data of Covid-19 deaths according to the date on which the death <em>occurred<\/em>. The <\/span><a href=\"https:\/\/bit.ly\/2N5cJAj\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Ministry reasoned<\/span><\/a><span style=\"font-weight: 400;\"> that the switch was necessary because the internationally-accepted system led to the publication of \u201ccases of laboratory results of deaths recorded weeks ago, but that count for the accounts of that day.\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The effect was a net reduction in reported Covid-19 deaths. According to <\/span><a href=\"https:\/\/bit.ly\/2UQHJbx\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">estimates by newspaper <\/span><i><span style=\"font-weight: 400;\">Folha de S\u00e3o Paulo,<\/span><\/i><\/a><span style=\"font-weight: 400;\"> the switch left at least 44% of coronavirus deaths out of official data publications, as Covid-19 confirmation often only becomes available post-mortem. The same analysis also points to political interference, noting that \u201c[t]he government started to talk about changing the publication methodology after Brazil broke consecutive [daily] records for deaths.\u201d<\/span><\/p>\n<p><a href=\"https:\/\/www.rioonwatch.org\/wp-content\/uploads\/2020\/06\/A-Question-of-Methodology_Divergent-Methods-and-Conflicting-Outcomes-in-Brazils-Covid-19-Coverage-5.jpeg\" target=\"_blank\" rel=\"noopener noreferrer\"><img fetchpriority=\"high\" decoding=\"async\" class=\"aligncenter wp-image-60292 \" title=\"Supreme Court Justice Alexandre de Moraes. Changes in <\/span><a href=\"https:\/\/bit.ly\/3hxdhx7\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">reporting methodology<\/span><\/a><span style=\"font-weight: 400;\"> shape the information available to health professionals, policy-makers and the general population, altering everyone&#8217;s ability to make informed decisions.<\/span><\/p>\n<h3><b>The Impacts of Case Definitions on Covid-19 Reporting Methodology<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">Reporting Covid-19 data starts with case counting. The WHO uses a <\/span><a href=\"https:\/\/bit.ly\/3hFOMO2\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">three-tier system for counting and reporting Covid-19 cases<\/span><\/a><span style=\"font-weight: 400;\">: suspected, probable, and confirmed, with the last tier defined as a \u201cperson with laboratory confirmation of Covid-19 infection, irrespective of clinical signs and symptoms.\u201d This is the only tier of the case reporting system that relies on laboratory test results to confirm the presence of the virus and is only one part of the data equation when it comes to providing data for effective policy-making.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">For the WHO&#8217;s five other definitions (three for suspected cases and two for probable cases), clinical observations of symptoms are required to confirm or deny the possibility or probability of the virus. In the context of the global shortages in testing capacity, the results of clinical observations become critical.<\/span><\/p>\n<p><a href=\"https:\/\/bit.ly\/30OuTi5\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Brazil uses a two-tier system<\/span><\/a><span style=\"font-weight: 400;\">, counting only suspected and confirmed cases. \u201cConfirmed\u201d cases, as the WHO recommends, require laboratory test confirmation. Health professionals and institutions from both public and private sector health units are required to report all suspected and confirmed cases to health authorities. However, in Brazil only confirmed cases are being published on official data dashboards.\u00a0<\/span><\/p>\n<p><span style=\"font-weight: 400;\">As only confirmed cases are published on official data dashboards, and cases can only be reported as confirmed by laboratory testing, then Brazil\u2019s testing capacity is crucial. However, Brazil has been <a href=\"https:\/\/glo.bo\/3fAbwgJ\" target=\"_blank\" rel=\"noopener noreferrer\">severely under-testing<\/a> since the beginning of the pandemic. <\/span><span style=\"font-weight: 400;\">The WHO recommends that countries conduct sufficient testing such that an average 5% of tests return positive daily\u2014Brazil averages a 36.7% positive rate. That is: only the most dramatic and visible of cases are being tested.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The report also points out that not all laboratories conducting tests are registered in the system from which the Ministry of Health receives test results, meaning that the Ministry is not accessing all available Covid-19 data.\u00a0<\/span><\/p>\n<figure id=\"attachment_60289\" aria-describedby=\"caption-attachment-60289\" style=\"width: 620px\" class=\"wp-caption aligncenter\"><a href=\"https:\/\/www.rioonwatch.org\/wp-content\/uploads\/2020\/06\/A-Question-of-Methodology_Divergent-Methods-and-Conflicting-Outcomes-in-Brazils-Covid-19-Coverage-3.jpg\" target=\"_blank\" rel=\"noopener noreferrer\"><img decoding=\"async\" class=\"wp-image-60289\" title=\"Covid-19 test average for every 100 thousand inhabitants - the rates take into consideration tests done up to 29 May 2020. Source: G1<\/figcaption><\/figure>\n<h3><b>Political Influence in Covid-19 Reporting Methodology<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">In a <\/span><a href=\"https:\/\/bit.ly\/2A8p9V7\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">recording of an April 22 cabinet meeting<\/span><\/a><span style=\"font-weight: 400;\">, President Jair Bolsonaro told ministers and members of the government to <\/span><a href=\"https:\/\/bit.ly\/3ene3u7\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">emphasize patient comorbidities when recording Covid-19 deaths<\/span><\/a><span style=\"font-weight: 400;\"> so as to decrease the official numbers being published as Covid-19.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The primary objective of the <\/span><a href=\"https:\/\/bit.ly\/2YaXZXh\" target=\"_blank\" rel=\"noopener noreferrer\"><i><span style=\"font-weight: 400;\">WHO International Guidelines for Certification and Classification (Coding) of Covid-19 as Cause of Death<\/span><\/i><\/a><span style=\"font-weight: 400;\">, is to \u201cidentify all deaths <\/span><span style=\"font-weight: 400;\">due to<\/span><span style=\"font-weight: 400;\"> Covid-19\u201d to facilitate an inclusive system for the practice of registering deaths for official publication:<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">A death due to Covid-19 is defined for surveillance purposes as a death resulting from a clinically compatible illness, in a probable or confirmed Covid-19 case, unless there is a clear alternative cause of death that cannot be related to Covid disease (e.g. trauma). There should be no period of complete recovery from Covid-19 between illness and death.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">The WHO\u2019s reporting system does not allow Covid-19 deaths to be attributed to comorbidities. Rather, it specifies a way of mapping the chain of clinical events to determine the causal sequence leading to death while clearly underlining the underlying cause of death. The system allows the medical history of the patient to be documented as accurately as possible throughout the course of the illness(es). If a patient has comorbidities, the system registers them in a separate section of the form. The change is relevant to the <a href=\"https:\/\/bit.ly\/3ene3u7\" target=\"_blank\" rel=\"noopener noreferrer\">president&#8217;s request<\/a> that &#8220;whoever has the right, the respective ministry&#8221; include less deaths due to Covid-19 in official reports. <span style=\"font-weight: 400;\">The guidelines state: \u201cIf, at the time of filling in the Death Certificate, the cause of death isn\u2019t confirmed as due to Covid-19, but there is a suspicion [it was], the doctor will write down \u201csuspected Covid-19\u201d in part 1 [for cause(s) of death].\u201d<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The Brazilian guidelines then explain that the \u201crecommendation for filling-in \u201csuspected Covid-19\u201d is international and aimed at capturing all possible deaths from the illness\u201d but that ultimately, in Brazil:<\/span><\/p>\n<blockquote><p><span style=\"font-weight: 400;\">The confirmation or ruling out of Covid-19 will remain the responsibility of the Municipal and\/or State Health Secretariats.<\/span><\/p><\/blockquote>\n<p><span style=\"font-weight: 400;\">The information that remains after this initial sorting process is then passed on to the Ministry of Health, who themselves \u201cconsolidate\u201d the data before publishing it each day. The Ministry of Health does not publish suspected Covid-19 deaths on its official data dashboards, but the sequencing of this kind of data mining demonstrates the range of options available to public authorities in shaping published figures to discount the full impact of the pandemic.<\/span><\/p>\n<h3><b>Best Practices from Belgium<\/b><\/h3>\n<p><span style=\"font-weight: 400;\">In what might be seen as a 180\u00b0 approach from Brazil, in Belgium, the public health institution <\/span><a href=\"https:\/\/bit.ly\/3dbUfc3\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Sciensano<\/span><\/a><span style=\"font-weight: 400;\">, in charge of the country&#8217;s Covid-19 response, uses a <\/span><a href=\"https:\/\/bit.ly\/2UTxjbb\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">simplified version<\/span><\/a><span style=\"font-weight: 400;\"> of the WHO <\/span><i><span style=\"font-weight: 400;\">Case definitions for surveillance<\/span><\/i><span style=\"font-weight: 400;\"> to allow for the highest rate of reporting to provide the maximum amount of data. Moreover, in Belgium, it is <\/span><a href=\"https:\/\/bit.ly\/2Y8NWlq\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">obligatory for health professionals and institutions to declare<\/span><\/a><span style=\"font-weight: 400;\"> \u201call possible cases.\u201d Sciensano then identifies \u201cthe number of cases, whether or not confirmed\u201d and \u201cthe number of deaths, suspected or confirmed\u201d for their <\/span><a href=\"https:\/\/bit.ly\/2YDm8o4\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">data collection and analysis<\/span><\/a><span style=\"font-weight: 400;\">. Belgium&#8217;s <a href=\"https:\/\/on.wsj.com\/3fT9rfZ\" target=\"_blank\" rel=\"noopener noreferrer\">approach has been touted<\/a> as that providing the <a href=\"https:\/\/wapo.st\/2AYQwlk\" target=\"_blank\" rel=\"noopener noreferrer\">most accurate accounting<\/a> of <a href=\"https:\/\/bit.ly\/3ecoYa9\" target=\"_blank\" rel=\"noopener noreferrer\">the virus<\/a>.<\/span><\/p>\n<p><a href=\"https:\/\/bit.ly\/3hDqK6r\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Sciensano publishes suspected Covid-19 cases<\/span><\/a><span style=\"font-weight: 400;\"> inside and outside hospitals in their official figures. This is because they consider suspected cases as important as confirmed cases. In the absence of full testing, <\/span><a href=\"https:\/\/bit.ly\/2BcNkSJ\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">counting suspected cases is the best proxy<\/span><\/a> to approach accuracy<span style=\"font-weight: 400;\">. Belgium has sought to avoid underestimating Covid-19 cases and deaths as a primary goal. <\/span><a href=\"https:\/\/bit.ly\/3hDqK6r\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">Sciensano considers<\/span><\/a><span style=\"font-weight: 400;\"> it \u201cgood standard practice\u201d to record confirmed and suspected cases, <\/span><a href=\"https:\/\/bit.ly\/3ecoYa9\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">following the recommendations<\/span><\/a><span style=\"font-weight: 400;\"> of the WHO and the European Center for Disease Prevention and Control.<\/span><\/p>\n<p><span style=\"font-weight: 400;\">The result of Belgium\u2019s inclusive counting and reporting methodology is that the country has <\/span><a href=\"https:\/\/bit.ly\/37IiFsz\" target=\"_blank\" rel=\"noopener noreferrer\"><span style=\"font-weight: 400;\">very high Covid-19 case and death rates<\/span><\/a><span style=\"font-weight: 400;\"> for the size of the population. The Prime Minister Sophie Wilm\u00e8s explains that \u201cBelgium has chosen the greatest transparency in communicating deaths linked to Covid-19.\u201d\u00a0<\/span><\/p>\n<p><a href=\"https:\/\/www.rioonwatch.org\/wp-content\/uploads\/2020\/06\/A-Question-of-Methodology_Divergent-Methods-and-Conflicting-Outcomes-in-Brazils-Covid-19-Coverage-7.png\" target=\"_blank\" rel=\"noopener noreferrer\"><img loading=\"lazy\" decoding=\"async\" class=\"aligncenter wp-image-60296 size-content\" title=\"Still from the videoed meeting in which President Jair Bolsonaro told ministers and members of the government to emphasize patient comorbidities when recording Covid-19 deaths.
